Below is a compendium of resources that have been developed by Florida Hospital (FH), or adapted from the Institute for Healthcare Improvement (IHI), in Florida Hospital's implementation of the Global Trigger Tool (GTT). These resources are available for anyone to use "as is" or to modify to suit to the organization's environment. Database
Description
Access database elements and specifications necessary for Global Trigger Tool data aggregation, tracking, trending, and reporting based on Institute for Healthcare Improvement (IHI) Global Trigger Tools (GTT) methodology (2007 ver).
Example Reports generated by the Patient Safety Officer from the data for optional distribution, for purposes of tracking and trending IHI-defined Adverse Event Rates.
Note: Report template assumes that the database is in a specific location – user/administrator should change this location.
Summarization of Trigger Tool worksheet findings following Chart Reviewers agreement and useful for discussion with Physician Authenticator. Once Harms are confirmed by the Physician, the Summary Report is the source for database entry. There are two optional forms provided.
